ACA vs. Private Health Insurance: What to Compare Before You Choose

“Private health insurance” can mean several different things, which is why comparing plans by monthly premium alone can be misleading. An ACA-compliant major medical plan, a medically underwritten private plan, and a fixed-indemnity or supplemental policy may all help with health-care costs, but they do not necessarily cover the same risks or work the same way.

Start with what kind of coverage it is

ACA-compliant plans follow federal requirements for essential health benefits and consumer protections. Depending on household income and eligibility, some people may also qualify for premium tax credits through the Marketplace.

Other private options may use medical underwriting, different benefit structures, narrower eligibility rules, or different limitations. Fixed-indemnity plans, for example, generally pay stated benefits for covered services rather than functioning like comprehensive major medical insurance.

Five things worth comparing

Provider access: Are your doctors and hospitals in network? Prescriptions: How are your medications covered? Cost sharing: What are the deductible, copays, coinsurance, and out-of-pocket exposure? Underwriting: Are health questions involved? Limitations: Are there exclusions, benefit caps, waiting periods, or services that are not covered?

When a lower premium is not the whole answer

A lower monthly premium can be attractive, but it should be weighed against the risk you are keeping. The right comparison asks what happens if you need a specialist, expensive prescription, surgery, hospitalization, or ongoing treatment—not just what the plan costs in a healthy month.

For self-employed clients

Self-employed individuals often have more plan-shopping responsibility because there is no employer choosing the benefit structure. That makes it especially important to compare ACA eligibility, available private options, provider networks, prescriptions, and the financial tradeoffs of each structure.
