MEDICARE SPECIAL NEEDS PLANS

Some Medicare plans are built around specific health or financial needs.

Medicare Special Needs Plans are Medicare Advantage plans for people who meet specific eligibility rules, including certain chronic conditions, institutional status, or qualifying Medicaid status. Eligibility is plan-specific, and the plan still has to fit the person’s doctors, prescriptions and budget.

What is a Medicare Special Needs Plan?

A Special Needs Plan, or SNP, limits enrollment to people who meet the plan’s approved eligibility criteria. SNPs can combine medical and prescription coverage and may include care-management or supplemental benefits designed around the population served.

What is the difference between a C-SNP and a D-SNP?

C-SNP D-SNP
For people with qualifying chronic or disabling conditions For people with Medicare and qualifying Medicaid status

Can diabetes qualify for a C-SNP?

Diabetes mellitus is one of the chronic-condition categories CMS permits C-SNPs to target. A person still has to meet the specific plan’s eligibility and verification requirements.

Can chronic heart failure qualify?

Chronic heart failure is also one of the chronic-condition categories CMS permits C-SNPs to target. Eligibility still depends on the specific plan and required confirmation.

Can stroke qualify for a C-SNP?

Yes. CMS lists stroke as a C-SNP chronic-condition category, and it separately allows certain C-SNPs to target a clinically linked grouping of stroke and cardiovascular disorders. That does not mean every C-SNP accepts every person with a history of stroke: the plan must actually target an applicable CMS-approved condition or grouping, and the plan’s eligibility and verification rules still apply.

Does everyone on Medicaid qualify for the same D-SNP?

No. D-SNP eligibility can vary with Medicaid status and plan rules, so the person’s exact eligibility and the plan’s service area must be checked.
